It does sound like eye trauma to me. It can be from a scratch, or sting from a coral or anemone, I have also seen this happen from ich outbreaks. I would not remove it. Feeding it well and soaking food in a liquid vitamin like Selcon, an antibiotic like SeaChem's KanaPlex, and garlic juice for taste will help. If it is a minor eye trauma it will clear up in a week or two.

Not dismissing the scratch from a fight, though, as mine get that periodically from sparring, too, but it usually clears up within a day or two at most.
